The cost to purchase the NCPDP Implementation Guides and Data Dictionaries is $650.00. With that price you also get an
NCPDP membership allowing you access to all standards updates, the member directory, work group meetings and numerous
other invaluable networking opportunities.
In addition to access to all NCPDP standards documents, NCPDP members are also allowed to share the standards documents with departments and personnel within that individual member’s company for business purposes. Sharing beyond this must be approved by NCPDP.

The "old" Universal Claim Form (aka "blue & white/version 3.2/2 part") will be available in limited supplies. This form may be referred to as "DAH 3-97" for the revision date or any of the previous names. We will monitor inventory to not have a surplus of the old forms 2003. The "version 5" Universal Claim Form is available and referred to as "DAH 2PT". The laser UCF is available and is referred to as "UCF L1".
